Mental Health Therapist – Behavioral Health

Job Overview

Provide integrated behavioral health services within a primary care setting, including mental health assessments, treatment planning, and individual and family therapy. This position works collaboratively with primary care providers and community resources to support the overall well-being of patients.

Key Responsibilities

• Conduct mental health assessments and use DSM-5 criteria to support accurate diagnosis and treatment.
• Develop person-centered treatment plans and provide evidence-based therapeutic interventions.
• Provide individual and family therapy and document care according to department standards.
• Collaborate with primary care providers, clinic staff, and outside resources to coordinate patient care.
• Connect patients with appropriate community resources and provide advocacy as needed.
• Participate in care team meetings, quality initiatives, and ongoing professional development.
• Maintain accurate documentation, productivity standards, reliable attendance, and patient confidentiality.
• Demonstrate compassion, integrity, collaboration, kindness, and professionalism in caring for those we serve.

Qualificiations

Required Qualifications

• Master's degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or a related field.
• Current licensure as a Licensed Clinical Social Worker (PCMSW, CMSW), Licensed Mental Health Practitioner (PLMPH, LMHP, LIMHP), or eligible provisional status.

Preferred Qualifications

• Two years of experience providing direct therapy.
